All Star T-shirt Orders

Please place one order per Division by entering your totals for each size below.

Print the page before you click Send, for your Division records.

Click the "Send in my order please!" button at the bottom of this page when you are finished.

Please e-mail or fax the Divisional Order Request ( the list of individuals ) ordering shirts and the sizes they ordered to:

Please bring a copy of this to the all Star meets.

Orders are due by Midnight on Sunday 7/24/2011 !!!

T-Shirts are $15 each, no matter what size. Shirts are 100% pre-shrunk cotton.
All orders will be delivered at Allstars. Bring a check (made out to MCDL).
No shirts will be distributed without payment.
